Clinic Policies

CANCELLATION POLICY                                        
  • When you schedule an appointment, we reserve that time exclusively for you. We value your time and thank you for showing us the same courtesy. We request 24 hour notification of any change to or cancellation of an appointment.

  • If less than 24 hour notice is provided, you will be charged $60 for private sessions and $25 for community acupuncture sessions.

  • If no notice of appointment cancellation is provided (no show/no call), it is our policy to charge the missed visit at the normal visit rate of $105 for private sessions and $30 for community sessions.

  • These charges will either be charged to the credit card on file, billed to you, or collected at your next appointment. If the appointment was purchased in a package, the missed appointment charge will be deducted from the remaining appointments in the package. However,  we recognize that emergencies and extenuating circumstances arise and those will be considered on an individual basis when applicable and fees may be waived. 

LATE POLICY  

  • If you are going to be 15 minutes or more late for your appointment, please call to inform us as soon as possible.  We will do our best to work you into the schedule, however if we are unable to see you, you will be rescheduled and a $60 (private session) or $30 (community session) fee will be charged to you. If a patient is late and treated, the appointment will be shortened and end according to the original start time of the appointment or according to an agreement between the practitioner and the patient. Late patients will be charged the full fee regardless of the length of the visit.​​

PAYMENT POLICY
  • Full payment is due at the time of service for all office visits and procedures unless pre-arrangements have been made

  • We accept cash, checks, and credit/debit/HSA/FSA cards.

  • This office is not a participating provider with any insurance carrier. However, if your insurance does cover acupuncture, we are happy to provide you receipts for you to  forward to your insurance company for reimbursement. 

  • Herbs/supplements cost is separate from appointment fee

  • Returned checks will incur a $25.00 fee

PRIVACY POLICY / REQUEST FOR MEDICAL RECORDS​​
  • All contact information will be kept confidential and may only be used for contact purposes, unless written consent is given. Our office follows HIPAA regulations for processing medical record releases at all times.  A signature form is required to process any/all requests for the release of records.
